To install, unpack files to /usr/share/smplayer/

Also you can install it for current user only(just unpack archive to ~/.config/smplayer/)

For activate theme, open preferences(Option->Preferences or just press Ctrl+P), select "Interface" on the left then choose icons in "Icon set" combo-box on the right.

A question though, why do I have only text at menubar (Open, Play, Video...) instead of icons like in your screenshot?

Hm, to my mind your toolbar is disabled. To enable it select "Options -> Toolbars -> Main toolbar" or just press F5 (default key shortcut for enable\disable main toolbar).

As for menubar (Open, Play, Video...) I haven't it in application window, because I use global menu (like in Mac OS X).
